Isolation (LOTO) Permit Book

The Isolation (LOTO) Permit Book is used by safety officers, maintenance supervisors, engineers, and authorised isolators to document and manage formal lockout tagout permits across maintenance, shutdown, and repair activities in facilities where hazardous energy isolation is a required control under WHS legislation. Under the WHS Regulations and the relevant Codes of Practice, workplaces with plant and equipment subject to unexpected energy release must have a documented isolation procedure, and a permit system provides the formal authorisation and accountability record that supports compliance with that obligation. The permit form covers task details including location, permit holder, description of work, and supporting permit references for JSA/SWMS, confined space, excavation, hot work, and working at height permits. The equipment isolation table records each isolation point, energy type, isolating person, checker, and tested-for-dead confirmation, and the risk assessment checklist covers competency, hazard identification, isolation testing, de-energisation, lock and tag application, guarding, barricading, safety observer, and worker briefing. The all-personnel sign-on and sign-off table records each worker's name, lock and tag number, on and off signatures, and times, ensuring every person working under the isolation is individually accounted for throughout the permit period. The permit closure section confirms machine guarding replaced, all locks and tags removed, and equipment returned to service, with sign-off by the person who must match the original isolator. It is used across manufacturing, mining, oil and gas, utilities, processing, and any industrial facility where formal LOTO permit systems are a central component of the maintenance safety management system.
