Who is this for

The High Voltage Access Permit Book is used by authorised HV operators, electrical engineers, switching officers, and safety officers to document and authorise access to high voltage apparatus and equipment for maintenance, testing, and switching operations. Under Australian electrical safety legislation and the relevant electricity network operator standards, access to high voltage plant must be formally authorised through a documented permit system that confirms isolation, earthing, safe working distance, and the competency and authorisation of all personnel before any work on or near high voltage equipment commences. The permit form covers task details including substation and site location, permit holder, HV apparatus and equipment ID, voltage level in kilovolts, start and end date and time, and description of work, alongside supporting permits for JSA/SWMS and a sanction to test permit number where earths are to be lifted for testing. The risk assessment checklist confirms that a full risk assessment has been completed, at least two authorised HV operators are participating in switching operations, all personnel are competent and authorised for HV access, the apparatus is treated as energised until isolated and proven de-energised, isolation points are identified and locked out, earthing is applied at all required points, a switching programme has been prepared and followed, safe working distance can be maintained, the area is barricaded and signposted, PPE including insulated gloves and arc-rated clothing is available and in date, test instruments are calibrated and rated for the voltage, and communication between operators and the isolation officer is established. The all-personnel sign-on table records each worker's name, HV authorisation number, signature, and in and out date and time, and the permit closure section confirms all earths and portable earthing equipment have been removed and accounted for, isolation locks and tags have been removed by the isolation officer, and the switching programme to re-energise has been completed and the apparatus confirmed safely energised.
