The permit covers project information, contractor details, person issuing the permit, estimated completion, and termination of permit, and includes six pre-penetration checks covering approved JHA attachment, sketch or photo of the penetration area, electrical and plumbing service identification and sign-off on services drawings, concrete scanning confirmation, and area clearance. The seven-day review cycle ensures the permit is reassessed regularly throughout a longer penetration programme, and the basic task steps section records prestart meeting, electrical and plumbing isolation and testing, work area barricading, and sign-on requirements. All personnel working under the permit must sign on to confirm they understand the permit requirements before work commences.
