It is used across commercial buildings, hospitals, aged care facilities, industrial plants, hotels, shopping centres, and any facility where sprinkler systems, fire hydrants, hose reels, suppression systems, or fire panels are installed and subject to periodic isolation. The form covers impairment type, fire watch requirements, job specific resources, notification obligations, and two critical points checklists for pre-impairment and post-reinstatement sign-off. The duplicate NCR format provides a copy for the person responsible for the impairment and a retained record for the building owner or facilities manager, with completed authorisations required to be kept for seven years for audit purposes.
