The front records the impairment tag number, device type, equipment isolated, controlling valve, out-of-service date and time, duration, person responsible, reason for impairment, notification number, authorising officer, and reinstatement details, while the reverse provides a structured checklist of required actions before, during, and after the impairment including completing risk assessments, alerting emergency management, keeping fire doors closed, shutting down hazardous operations, verifying protection has been restored, and notifying emergency management on reinstatement. The perforated emergency management copy at the base of the front is detached and placed on the impairment board, ensuring the emergency management team has a record of all active impairments at all times. The high-visibility red construction ensures the tag is immediately identifiable on any fire protection device.
