The corrective action plan section applies the hierarchy of controls, prompting the reporter to consider elimination, substitution, isolation, and risk reduction before PPE and exposure reduction, and records the recommended actions alongside who is responsible and by when. The manager or supervisor section records whether control measures have been implemented and confirmed effective, whether a follow-up risk assessment is required, and whether entry onto the site risk register is required, with sign-off by the actioning manager.
